How much do office visits cost?
All In-Network visits are subject to copays, coinsurance and deductibles. Please obtain detailed information from your insurance company regarding plan details and your out of pocket responsibility.
What do I need to bring to the appointment?
- Patient Visit Form (optional)
- Insurance card
- Drivers license, passport or a valid form of identification
- Reports, previous x-rays, MRI or CT scans and any other relevant patient information
- List of medications currently taken
What should I expect during my first visit?
Please arrive 30 minutes prior to your scheduled clinic visit to allow time for sign-in and the processing of insurance and billing information.
When you arrive, the patient coordinator will help you check in. They will collect or have you complete the patient visit form, and make copies of your insurance card(s) and ID. Be sure to give them any x-ray films, MRI/CT disks or additional diagnostic paperwork that you brought with you so they can be reviewed by Dr. Feldman and his team prior to your consult.
Do you treat patients outside of the West Palm Beach, Florida area?
Patients can be seen at our satellite office in New York City, NY for consult and follow-up visits only. All surgical procedures and post-op appointments take place at The Paley Institute in West Palm Beach, FL.

Are my medical records kept private and confidential?
Our office is designed to provide maximum protection of patient medical records. Access to medical records is restricted to the office staff for medical and office related functions. However, medical information obtained during the course of examinations may be released with your permission to personal physicians, referral physicians, and any other health care providers you authorize. Medical information may also be shared with your insurance carrier in situations where we feel it’s necessary and in your best interest.
